How To Apply

APPLYING AT FOA IS SIMPLE.
JUST FOLLOW THE STEPS BELOW!

First-Time Applicants


1.) Contact the school to set up a meeting with Mrs. Sierra, the Principal of FOA.

2.) She will give you the necessary documents to fill out.

3.) When finished, drop off the form and required paperwork at the office or hold onto them until your next meeting with Mrs. Sierra.

Renewal Applicants


1.) Download the Renewal Registration Form.

IMPORTANT

This form is only for RETURNING students.
New students need to follow the steps listed in the First-Time Applicants section.

2.) Fill out the form to the best of your ability.

3.) You can either drop off the form, and required paperwork, at the office, or you can submit them electronically through our FILE UPLOAD page.
